STEP ONE:

• Drug Test Consent form

• Emergency Contact form

• Equipment Terms form

• Flu Immunization Form

• Gratuitous Agreement

• Policies and Procedures

• Student Registration Form

• Student Handbook (please review)

• Student Handbook Acknowledgement and Receipt

• TB Immunization Form - must be recent within the last year

• Please send a photo so we can include a welcome in our newsletter for our staff. 

• Submit a copy of your Driver's License.

• Submit copy of SS Card or Birth Certificate.

STEP TWO: 

To prepare for your rotation, we ask that you watch these three brief videos on Federally Qualified Health  Centers and InterProfessional Education and submit a short reflection to this Google form:  PRE-CLINICAL REFLECTION
